You will need your Highmark member ID found on the front of your Highmark member ID card and your date of birth. Enter your Highmark member ID number (12 digits) in the Member ID field, enter your date of birth (MMDDYY) in the Date of Birth field, and enter your first and last name in the Name field, exactly as it appears on your member ID card.
